The paper applies Decision Accounting to global regulatory convergence in operational resilience and individual accountability. It proves that retrospective regimes (DORA, SMCR, FAR, CPS 230, IAC) leave a system-welfare failure intact, and proposes mandatory Field 17 system-welfare accounting as the missing prospective gate.
